Plus, it’s FREE and open to all adult and pediatric palliative care programs across the continuum of care: hospital, home, office/clinic, and long-term care programs. Based on feedback from participating palliative care programs and the field, this year we have made significant changes and the Registry now includes two new and improved community surveys, Patient’s Home and Long-term Care, and a pediatric palliative care module in the hospital survey. By entering your data in the Registry, you will be able to:
